TI notification
The employee's manager should be notified of the death by the employee's family. If the employee's family does not know how to contact his or her manager or has other HR or payroll questions related to an employee's death, they should call HR Connect at 888-660-1411 and select the option for HR & Payroll.

In addition, a member of the employee's family should report the death to TI's benefits administrator by calling HR Connect at 888-660-1411 and selecting the option for the TI Benefits Center. The family member will then be connected with the TI survivor support service (see below).

TI provides help in processing death-related health and financial benefits for active-status employees, their spouses/domestic partners and dependents/executors. This service will take you step-by-step through the process. Read the details at the Survivor Services Benefit page.

TI health benefits
For detailed information on TI benefits that provide income protection in the event of your death, such as Accidental Death and Dismemberment (AD&D) protection, life insurance, or business travel insurance plans, review the Health Benefits Guide. This guide is available on the Health Benefits* website under the menu item "Benefit Guides" or on NetBenefits* under the Health & Insurance tab; select the "Reference Library" link at the bottom of the page and refer to the Summary Plan Description. For questions related to these benefits, contact HR Connect at 888-660-1411 and select the option for the TI Benefits Center.

The employee's eligible dependents may be able to continue health coverage for up to 18 months under the Consolidated Omnibus Budget Reconciliation Act of 1985 (COBRA). If so, COBRA election information will be sent under separate cover. If COBRA coverage is elected, surviving dependents will have access to a free employee assistance program (EAP) that can provide counseling resources to help cope with the loss. The EAP uses a solution-oriented therapy approach to problem resolution. You can find additional information on the EAP* Web site or by calling HR Connect at 888-660-1411 and selecting the option for employee assistance programs. You can also call Magellan directly at 800-888-CARE (2273).

Employee Assistance Program (EAP) and Work-Life Resources
TI offers a free employee assistance program through Magellan that can help you through changes or issues at any stage of your life, whether they be work-related or personal. The EAP provides free counseling sessions using a solution-oriented therapy approach to problem resolution and a comprehensive online library of work-life resources including articles, tools, webinars/podcasts, calculators, self-assessments and more. Work-Life topics include childcare, senior care, daily needs, education, adoption and special needs. You can find additional information on the EAP* website or by calling TI HR Connect at 888-660-1411 and selecting the option for employee assistance programs, then option 1 to speak to someone in person. You can also call Magellan directly at 800-888-CARE (2273).

Stock options
The death of an employee can can impact nonqualified stock options (NQSOs) and restricted stock units (RSUs). If the employee has stock awards, please review the termination provisions* on the TI Equity Programs* Web site for each type of grant owned by the employee. This site also has answers to many frequently asked questions and contact information for the TI Equity Administrator and the UBS One Source* service center.

Employee Stock Purchase Plan (ESPP)
Enrollment in a current Employee Stock Purchase Plan (ESPP) offering will cancel upon death and the account balance will be refunded. Please visit the ESPP* Web site for more information on this program. Use the UBS One Source* Web site to monitor and take action on your employee stock purchases.
